Symptom narrative

Patient has not reported any adverse symptoms/sign. This was incident was discovered when I, Vaccine Coordinator pulled vaccine box and asked staff if any doses were administered today as the expiration date was yesterday 12/4/25. It was discovered that 1 dose was administered today. Family/patient will be contacted to redo vaccine as soon as possible. Internal report and leadership has been notified of incident.
