Symptom narrative

Pt requested and received a dose of Arexvy today (12/5/25). After he received the vaccine he asked how often one is to get the vaccine and I informed him that it is just a single lifetime dose. He then informed me he had a dose in January 2024 at a Pharmacy in another state. He was under the impression he was due for another dose. I did carefully review his online database and it did not appear before the vaccine. I informed him that we would report the incident to VAERS.
