Symptom narrative

Only adjuvant administered; Inappropriate dose of vaccine administered; This non-serious case was reported by a physician via call center representative and described the occurrence of inappropriate preparation of medication in a 63-year-old female patient who received Herpes zoster (Shingrix) (batch number G77F3, expiry date 05-JUN-2027) and (batch number 4C3RB, expiry date 05-JUN-2027) for prophylaxis. Previously administered products included Shingrix (1st dose received on 03-SEP-2025, batch number-5Y49C and expiry date-04-APR-2027). On 03-DEC-2025, the patient received the 2nd dose of Shingrix. On 03-DEC-2025, an unknown time after receiving Shingrix, the patient experienced inappropriate preparation of medication (Verbatim: Only adjuvant administered) and inappropriate dose of vaccine administered (Verbatim: Inappropriate dose of vaccine administered). The outcome of the inappropriate preparation of medication and inappropriate dose of vaccine administered were not applicable. Additional Information: GSK Receipt Date: 03-DEC-2025 Physician explained that one of their nurses accidentally gave only the adjuvant portion of Shingrix to a patient without mixing. The reporter consented to follow up.
