Symptom narrative

Patient's upper arm was swollen and red from the top of the upper arm to the the elbow covering the entire area. Patient agreed to proceed to Urgent care facility for evaluation by a physician. Patient contact me after and stated physician believed was just a severe site reaction and wanted to follow a watch and wait treatment strategy. Patient was instructed my physician if the reaction site began to spread around the back side of the arm -he should proceed to the Emergency Department as soon as possible.
