Duplicate vaccination given. Patient came in pharmacy on 12/5/25 requesting RSV per MD recommendation. I checked state shots and there was no history of RSV vaccination. Vaccine was administered. Pharmacy Customer Experience emailed the pharmacy on 12/6/25 indicating the patient had received Arexvy on 12/17/23. Patient was contacted and no issues reported.
