prescription was received electronically on 8/9/2025. Prescription was sent for human papillomavirus vaccine, 9-valent, but was inputted and 4-pointed for Shingrix. There are no DURs that show up for verifying patient age who was under 50 years old. Patient was notified today that she received the incorrect vaccine. Patient received Gardasil as her first dose which was the intended vaccine.
