Symptom narrative

Shortly after administration of the vaccine, I passed out in the doctor's office and had to be wheel-chaired to another room to recover. In the following days, and from 11/24 to 11/29 especially, the fatigue turned severe, including sleeping most of the day on 11/25. I also experienced confusion, weakness, muscle soreness throughout my body, dizziness, disorientation, and imbalance, making it difficult for me to even climb in and out of the shower tub without falling. I could not drive most days. I also have a severe dry cough. Dizziness, light-headedness, and the cough persist as of the date of this form.
