we gave a patient an expired Pediarix that expired on 10-31-25; This non-serious case was reported by a pharmacist via call center representative and described the occurrence of expired vaccine used in a male patient who received DTPa-HBV-IPV (Pediarix) (expiry date 31-OCT-2025) for prophylaxis. On 03-DEC-2025, the patient received the 2nd dose of Pediarix. On 03-DEC-2025, an unknown time after receiving Pediarix, the patient experienced expired vaccine used (Verbatim: we gave a patient an expired Pediarix that expired on 10-31-25). The outcome of the expired vaccine used was not applicable. This report is made by GSK without prejudice and does not imply any admission or liability for the incident or its consequences.
