Incomplete course of vaccination

Symptom narrative

received the first dose of Shingrix 11/19/19, forgot to get the 2nd dose; This non-serious case was reported by a nurse via call center representative and described the occurrence of incomplete course of vaccination in a 56-year-old male patient who did not receive Herpes zoster (Shingrix) for prophylaxis. Previously administered products included Shingrix (1st dose received on 19-NOV-2019 from batch number AC79X). The patient did not receive the 2nd dose of Shingrix. The patient experienced incomplete course of vaccination (Verbatim: received the first dose of Shingrix 11/19/19, forgot to get the 2nd dose). The outcome of the incomplete course of vaccination was not applicable. Additional Information: GSK Receipt Date: 04-DEC-2025 The reporter wanted to know whether the patient had to restart the series or if they could have proceeded with the second dose today (on the day of reporting). The vaccine administration facility was the same as primary reporter. The batch number was not provided, and unable to contact the reporter.
