Symptom narrative

Menveo vaccine administered to patient (12/02) was not properly reconstituted according to manufacturer instructions. The lyophilized MenA component was reconstituted with sterile water instead of the required liquid Menveo vial (MenCYW-135). The liquid Menveo vial was not used during preparation.
