Symptom narrative

Rash on both cheeks. No signs of shortness of breath, tongue swelling or mouth swelling. No rash on body on injection site. Dr. notified. He assessed patient. Prescription for benadryl provided. When followed up with mom. She advised that the rash was going away. No worsening symptoms. VAERS form faxed
