Symptom narrative

12/1/2025 I received orders from attending physician doctor for pediarix and flu in system. I then drew in error Kinrix. Pt received one vaccine in each leg. Doctor assisted by holding pt arms. Pt did not cry and tolerated well. On 12/5/2025 during vaccine reconciliation it was identified that Pediarix was ordered and Kinrix was given. I reported the error to Doctor. She advised that the vaccine would not count in the series but would not do harm to the pt. She directed me to notify the patients parent and have them return to receive the correct vaccine. See telephone note regarding parental notification. Practice administrator was notified and error report will submitted to compliance.
