Symptom narrative

At the time of vaccination on 11/8/2025 the patient received only 1 vial of the 2-vials required. In error, only vial #1 (MenCYW-135 Liquid Component) was administered. Vial #2 (MenA Lyophilized Conjugate Component) was not reconstituted nor administered to the patient. The error was discovered on 12/8/2025, during a review of the fridge inventory. The patient was contacted and agreed to return on 12/9/2025 to receive the full vaccination per manufacturers recommendations. No adverse effects to the first partial dose were reported.
