Resident was given the PCV20 and High dose flu this AM, this nurse put orders in, gave vaccine and documented in immunization tab. Orders were not yet confirmed. the charge nurse saw the unconfirmed orders and confirmed them, then gave the shots again. Resident did not remember being given the shots earlier and bandaids were gone from her arms. The IP, which is this nurse, gives the vaccines for everything but the RSV and Shingles, floor nurses do those as they are not stock supply. IP forgot to ask charge nurse to verify order so she could check off the MAR to avoid this happening. Normally IP asks the charge nurse to verify order as I give the shots.
A-fib, HTN, heart failure, hyperlipidemia, ASHD, PVD, dementia, traumatic ischemia of muscle
vitamin d, maalox as needed, MVI with minerals, vitamin b12, tylenol as needed
