Patient came to pharmacy to show us that she had a vaccine reaction and a necrotic wound was in place of the vaccines she received and is going to wound care now. When the spot appeared about 1 week after vaccination, she saw her PCP and was given an antibiotic (doxycycline). She has since finished that antibiotic and the wound had turned black. She was then referred to wound care at the hospital. She showed me pictures of the wound on her phone from then and how it looked after wound care today and was improving. Patient had requested the vaccines both be given in her right arm since she had gotten her flu shot at her PCP office that morning. She received a Tdap and RSV vaccine in the same arm, approximately 1.5 inches apart and in 2 separate barriers for vaccines. All precautions and PPE measures were followed at time of vaccination. Informed patient I would report to VAERS, describing it as a necrotic wound as the tissue was black in the center and approximately 1 inch wide and 2.5 inches long.
