Symptom narrative

Patient in clinic for annual wellness visit along with younger sibling. Flu vaccine was ordered for patient and a Vaxelis was ordered on sibling. Clinical staff had brought both vaccines in the room with her and had thought that she grabbed the flu vaccine to administer to Patient. The moment she realized her error, she stopped and, after properly disposing of used vaccine, came to let clinical supervisor and child's provider of the error. Provider returned to the room and examined child as well as explained to mother the error. Two phone conversations the following day with mother regarding her child and assuring if needing to be seen and monitored they can call our office at any time. During the time of the phone call, typical vaccine reactions were noted - soreness in leg of vaccination site and a fever of 100.4.
