No adverse event; they administered a VAQTA vaccine instead of the intended Influenza Vaccine.; patient has already completed the 2 dose series and this was her 3rd 0.5ML dose; This spontaneous report was received from a medical Assistant and refers to a(n) 7-year-old female patient. The patient's medical history, concurrent conditions and concomitant therapies were not reported. On unknown dates, the patient was vaccinated with 2 doses of Hepatitis A Vaccine, Inactivated (VAQTA), Suspension for injection, (lot # and expiration date were not reported) for Prophylaxis. On 14-Nov-2025, the patient was administered a third dose of Hepatitis A Vaccine, Inactivated (VAQTA) (lot #Z010429, expiration date: 09-Oct-2026), dose number 3, 0.5 mL, Suspension for injection vaccine instead of the intended Influenza Vaccine (Wrong vaccine administered, Extra dose administered). No adverse event was reported.
