The patient has no symptoms; Pharmacist calling to report an AE in which a 4-month-old female child was inadvertently administered GARDASIL 9 instead of PNEUMOVAX 23. The patient has no symptoms. No other AE/No PQC.; This spontaneous report was received from a pharmacist and refers to a(n) 4-month-old female patient. The patient's medical history, concurrent conditions, and concomitant therapies were not reported. On 18-Nov-2025, the patient was inadvertently vaccinated with Human Papillomavirus 9-valent Vaccine, Recombinant (GARDASIL 9), lot #Y020588, expiration date: 11-Aug-2027, 0.5 mL (0.5mL once) (formulation, exact dose, route of administration and anatomical location were not provided) for prophylaxis instead of Pneumococcal Vaccine, Polyvalent (23-valent) Solution for injection (PNEUMOVAX 23) (wrong product administered). No additional adverse event reported (no adverse event).
