On 01-Oct-2025, the patient started therapy with Measles, Mumps, Rubella and Varicella (Oka-Merck) Virus Vaccine Live (PROQUAD), (lot #Z012062, expiration date: 18-Jan-2027), dose number 1, (unknown by reporter /1st dose) reconstituted with sterile diluent (BAXTER STERILE DILUENT), (lot # and expiration date were not reported). The vaccine underwent temperature excursion (Temperature: 5.2C; Time frame: 69 hours 0 minutes 0 seconds). There was no previous temperature excursion. No adverse event was reported. Follow-up information received from an Other health professional on 14-NOV-2025. Vaccine was received on a Friday around 1 pm. The Medical Assistant (M.A.) who received the vaccine placed it in the refrigerator instead of the freezer. The reporter found this vaccine around 10 am on Monday morning and the reporter immediately put vaccine into the freezer. The reporter called and a report was made. The temperature excursion did not occur as a result of a reading from a Digital Data Logger.
