Shortly after receiving his influenza and HPV vaccinations patient had what was consistent with a brief vasovagal syncopal episode. He was sitting in a chair in the exam room when it occurred. He slumped down in his seat a bit first and then fell forward and hit the center of his forehead on the floor (from a distance of about 2 feet in height). This was witnessed by MA, [name withheld], and Patient's father. I was called into the exam room immediately and he regained consciousness within a few seconds. He spent some time laying on the floor before moving to a seated position on the floor and then eventually to a seated position in a chair. He was given some apple juice (drank two pouches) and quickly returned to his baseline. His neurological exam was intact. He did have a small superficial hematoma (about 2 cm diameter) with no depression or bony step off on his forehead, just right of center, near his hairline. His vital signs were checked twice and were stable. He demonstrated excellent cognition and memory. Reported a very mild pain to the hematoma on his forehead but denied headache, nausea, dizziness. In total he was monitored for about 30 mins after the vasovagal episode and appeared stable and safe for discharge home. We discussed supportive measures at length -- hydration, electrolytes, proper nutrition, proper sleep and rest, return to activity in a graded fashion (no skate boarding today). Discussed if he develops further syncope, worsening headache, vomiting, vision change, mental status change, lethargy, dizziness, confusion etc please contact our team (family aware of on call team/RN team) immediately and seek medical attention. Patient did state he slept late this morning and had not eaten breakfast prior to his arrival and that he was feeling mildly anxious prior to the vaccine.
