Symptom narrative

Patient presented with parent to workplace flu clinic. Guardian requested and consent to have child receive flu vaccine. After clinic hours, staff were entering vaccinations into state vaccine database when it was noted that the patient had already received a flu shot on 9/26/25 at [name withheld] Clinic. Guardian was called twice and emailed once to clarify the matter, however, no one ever called staff back. Unable to gather further data. It is assumed that patient accidentally received an extra, unnecessary flu vaccine this season.
