Symptom narrative

Patient presented to the pharmacy on 12/4/25 to our pharmacy after getting the above vaccines on 11/20. She stated that after getting these vaccines she had to go to the emergency room the next day due to nausea, achiness, and trouble breathing. She stated that the hospital admitted her overnight and gave her medication for nausea and breathing treatments. She was released after the breathing treatments and has been doing fine since. She stated she was very sick.
