Patient received a flu vaccine at the visit. Along with the other shots listed above. After the shots were given, the father was given the VIS sheet and asked if the flu shot was given when the nurse stated yes, dad stated that patient had already received the flu vaccine at pharmacy, I went in and apologized to Dad for the error. After the error was made, I did see that the father had documented in our consent that a flu vaccine was given at pharmacy. We never received documentation from pharmacy at the flu vaccine was given. Patient had no concern concerning symptoms after the vaccine and father had no further questions.
