Patient initially received a Fluad vaccine on 10/8/25. Patient then reported for an additional flu vaccine on 11/12 and was given an afluria vaccine. Patient did not state that she had already received the vaccine on 10/8/25- patient must have forgotten and billing is not done until after the vaccinations are given.
