Symptom narrative

Child was given extra dose of polio vaccine as error was made due to not recognizing polio was already in the Kinrix Dtap. Vaccine error took place on 12/08/2025 at approx. 1630. Symptoms of bilateral arm tenderness started at approx. 2030 on 12/08/2025. Increased discomfort to arms the morning of 12/09/2025. Mother gave the child Tylenol and child went to school. Child returned home from school at 1600 on 12/09/2025 and reported his arms "felt much better".
