Symptom narrative

Employee received vaccine in medical office, then returned to his work department. Approximately 15 minutes later he informed medical staff that he had developed numbness in his left upper leg. I called our center doctor for advise, he suggested medical assessment at an ER as soon as possible. Phone call made to [withheld] for mandatory report of on site incident. During this call symptoms progressed to numbness in left foot, then left arm moving up neck and head over the course of about 1 hour post vaccination. At this time we chose to have a medical staff member take him to the closest ER. Employee was taken back right away to be assessed.
