Symptom narrative

Patient received vaccination in right deltoid on Friday 12/6/25 after being discharged from hospital per the patient's daughter. On Monday 12/9/25 the patient's daughter called and said we gave her mother a shot too low and described the area as red, swollen and tender. Patient's daughter visited pharmacy and provided a picture of the arm with a red swollen area 1-2 inches above the elbow.
