Symptom narrative

During an off-site flu vaccine event, patient's grandmother requested we give patient a flu shot. Flu shot was administered. The next week when staff was entering flu shot's into State Registry it was discovered that patient had received a flu shot on 10/29/2025 at her doctor's office. She was given Fluzone Trivalent, PF. No adverse reaction were observed by health department staff.
