An 86-year-old male patient received BNT162b2 omicron (lp.8.1) (COMIRNATY (2025-2026 FORMULA)), in Sep2025 as dose 1, single (Batch/Lot number: unknown) at the age of 86 years, in left arm for covid-19 immunisation. The patient's relevant medical history included: "COVID-19 INFECTION", start date: Dec2019 (unspecified if ongoing), notes: in around holiday of 2019 and again in 2020; "blood pressure/Blood pressure control" (unspecified if ongoing); "covid long hauler" (unspecified if ongoing). Concomitant medication(s) included: FLU VACCINE taken for immunisation, in Sep2025 as dose number unknown, single.; LOSARTAN taken for hypertension. Vaccination history included: Moderna shot vaccine (DOSE 1), for COVID-19 immunisation; Moderna shot vaccine (DOSE 2), for COVID-19 immunisation, reaction(s): "He had a reaction"; Bnt162b2 (DOSE NUMBER UNKNOWN), for COVID-19 immunisation. The patient underwent the following laboratory tests and procedures: lab work: (2025) Not allergic to any vaccine, food or other product. Clinical Course: During call, he stated that he was really very hard of hearing. No Lab test performed. Not the past two weeks, about three or four weeks ago, he was in emergency, he saw 5 different doctors and go in the emergency ward. He had short of breath and trouble breathing. If he stand up on the feet and he walk his blood pressure, it goes up to around 200, if he sit down and relax in about four or five minutes, he could get the blood pressure back down to like 140, something like that, but anytime on his feet the blood pressure goes sky high and he had problem breathing. His doctor told him that he was covid long hauler. He had the covid in around holiday of 2019 and 2020. He had a really bad, no one even know what it was back then, so he go along and then about probably March, He got the shot, it was the Moderna shot vaccine the number one and then a month later, about three four weeks later they had him come back later and to get the second one, when he got the second one, he had a reaction, and then they said do not take it anymore, so then they came out with the Pfizer, all of the Pfizer shots vaccine he got every year since Pfizer came out with them and he was doing okay until he had this last shot on 28Sep2025 or 29Sep2025 and then he had it, he has been messed up ever since. He still has it, it has not gone away. When clarified if company vaccine was prescribed by any doctor, he stated no, it was the annual thing. He did not go particularly for that, he went for a dermatologist appointment, and they mentioned he would want to get the Pfizer shot, like covid 19 and said, yes, he was here might as well. The date of event was about one or two days later. He stated that he went through emergency when his blood pressure was around 200, 205, then he had seen 5 different doctors, they do not know what to tell him. The time of vaccination was around 11, 12 o'clock in the middle of the day. No other vaccine was taken on same day as company vaccine. Any other vaccine 4 weeks prior to company vaccine: he had a Flu shot probably, may be two, three weeks before. He was not allergic to any vaccine, food or other product, they checked him for that with lab work. He mentioned that he received COVID vaccine on 28th or 29th of Sep2025 and has been really sick. He had covid 19 vaccine, was having blood pressure fluctuations concerns. When he stand up on his feet the blood pressure goes sky high, if he sat down in the recliner and give it 5 to 10 minutes his blood pressure will go back to normal. It was hard to take a shower even and could not do anything. He had loss of appetite, feel lousy, and had no energy. The information on the batch/lot number for BNT162b2 omicron (lp.8.1) will be requested and submitted if and when received.
