Symptom narrative

Mom called to report that vaccines were all given in the same arm. Mom said 3 vaccines were given in the same arm (left deltoid). According to MMRV documentation states that was given in right deltoid. The other vaccine Pneumococcal documented as given in the Left Deltoid. Left deltoid is red raised and hard. Doctor appointment was scheduled to rule out infection vs reaction.
