he cut himself on the broken glass; he cut himself on the broken glass; he cut himself on the broken glass; his fingers were bleeding on his right thumb and right middle finger; finger flange had completely broken off and there was exposed broken glass on the syringe; This Spontaneous report was received from Physician via Company representative and referred to himself as a Male patient of unknown age. The patient's medical history was not reported. The patient's past medications were not reported. The patient's concomitant medications were not reported. On an unknown date, the physician was going to administer Human Papillomavirus 9-valent Vaccine, Recombinant (GARDASIL 9) (Batch/Lot Number: Z008262, Expiration date: 22-Feb-2027). While attaching the needle to the pre-filled syringe, the physician noticed that his fingers were bleeding on his right thumb and right middle finger (skin haemorrhage). Physician stated that the finger flange had completely broken off into a separate piece (syringe issue), and there was exposed broken glass on the syringe. Physician did not realize the glass was broken and placed his fingers on the area and cut his fingers (skin laceration, injury associated with device, accident at work). Confirmed the dose and other doses from the complaint box had not been administered to patients. Action taken with Human Papillomavirus 9-valent Vaccine, Recombinant (GARDASIL 9) was not applicable. Outcome of the events skin haemorrhage, skin laceration and accident at work was unknown. Causality assessment was not provided.
