Symptom narrative

On 12/10/25 patient was inadvertently given the MMRV instead of the Varicella. No treatment was needed. NCIR helpdesk was contacted by the facility for further advice. Parent was notified about the incident and advice was communicated. Patient had no abnormal symptoms at the time of speaking with mom (6hrs between vaccine and phone call).
