Pt came into office to receive a needed booster of Tdap. Tdap was administered and stayed for the suggested monitoring time. Patient returned to office 25 minutes later saying she was not feeling well and had what felt like a lump in her throat. Seated patient and began taking vitals at 1620. Blood pressure was significantly out of range at 165/130, Pulse-70, 02-99 and Respirations- 24. Patient stated she felt as though she was going to pass out. Laid pt down and elevated feet. After a few minutes, pt reported feeling better. Vitals were again taken at 1627 and found to be in normal range with BP-120/73, P-63, 02-98. Stated she could still feel a lump in her throat. Administered 25mg Benadryl. After a few minutes, pt stated she wanted to sit up in chair. After only 1-2 minutes, she stated she was feeling sick again. At this time her sister had arrived and provided pt's inhaler she uses for asthma attacks and patient self administered her inhaler. She was vigorously coughing and still felt a large lump in her throat. Vitals were taken at 1635 and were 162/109, P-70, 02-98. Suggested calling an ambulance but pt refused and said she would have her sister drive her to Urgent Care. Pt was released to the care of her sister for transport to Urgent Care at 1645. Called Urgent Care and gave report to Nurse including vitals and medication administration.
