Symptom narrative

LVN administered the second dose of flu which was not needed. Nurse did not repeat back the vaccines prior to administering them to patient. Patient received a dose on 12/5/25 and 10/4/25 however already received a 2-dose series in the past so 2nd dose was not needed for this season.
