Symptom narrative

Two injections were given in each arm simultaneously by 2 nurses. Immediately following third and fourth shots, patient became pale and had a syncopal episode. Patient was placed supine on exam table with knees bent. Patient was coherent about 20 seconds after being unresponsive. An ice pack was placed on neck per patient's request along with being offered water. She c/o being hot, light headed, and nauseous. Patient also c/o stiff hands. Patient was able to ambulate without assistance within 15 minutes after syncopal event.
