Symptom narrative

Pt stated history of localized swelling to arm after a tetanus shot, and that she got a "allergy shot" from a doctor at that time. Pt in clinic for Tdap for school requirement; although planned to refer pt to PCP, dose inadvertently given. Pt kept in waiting room for 15 minutes after injection with no symptoms noted. Pt advised to seek emergency medical attention at the ER if any difficulty breathing or worrisome symptoms appear.
