Symptom narrative

Pt's spouse reports on 12/11/25 that the patient is experiencing pain and very limited motion in her left arm and has been since the date of the vaccine. Spouse said that the patient's provider stated that they believe the patient received the vaccine too high in the shoulder joint (SIRVA). Pt reports having symptoms as of today 12/11/25 and has been since date that the vaccine was administered.
