Symptom narrative

Client states that she has an itchy, red rash that started last night on her left arm where she had the YF vaccine. Client denies any other symptoms and no fever. Client advised to apply cool pack to arm and that she may take a Benadryl. If she develops a fever over 100F or developed any other symptoms was instructed to go to the ER.
