Symptom narrative

17 patients were accidentally administered Fluzone Southern Hemisphere (SH) Influenza 2025 Vaccine instead of the regular Fluzone for Seasonal Influenza 2025-2026 Vaccine. Unexpired SH Influenza was stocked in the fridge at the time of administration. Error occurred due to the busy environment of the workplace, mixed with the similar appearance of the SH and NH Influenza packaging and preloaded syringes. A brief check of the vaccine box did not immediately reveal that the incorrect box had been selected for vaccine delivery. Boxes of both vaccines look almost identical, please look to company to change packaging for SH vaccine.
