Patient was brought to our clinic accompanied by an employee. Patient was given a Moderna Spikevax COVID vaccine at our office. Employee had taken him to pharmacy before coming to our office and he received a Comirnaty COVID vaccine. The Comirnaty vaccine was not in the state wide registry.
