Symptom narrative

Patient is a 55 year old female here for her initial shingles vaccine. Education provided thoroughly, patient stated understanding. Vaccine administered and patient tolerated well. Patient stayed in my office with me for monitoring post vaccine. Vaccine was given at 1229, about 10 minutes post vaccine patient stated "is it normal to feel kind of like biofreeze in my legs on the right side? and also felt like a cool sensation to my chest." i did state this wasn't normal and asked if she was feeling short of breath or any tickle in her throat, patient denied these feelings. Patient then states she feels like her eye is shutting but it is still open on exam. Patient also states she felt tingly on the right side of her face as well as her eye still bothering her and now her ear was feeling hot. Staff also checked for stroke signs, we had her squeeze hands bilaterally which was normal, patient was able to smile with no issue or drooping, and she was able to walk with no abnormal gait. At this time we got vitals on the patient. 98.7 temp, 98 oxygen, 87 pulse, 16 Respirations, blood pressure 166/68. 911 was called. When ems arrived they did an evaluation and took vitals. Patient declined going with EMS and signed refusal with transportation via ambulance and is going to go to the ER via her husband. Patient called husband to transport to the ER, patient is still in my office for monitoring. We took blood pressure again at 1326 and it was 170/114. Patients husband arrived about 1338 to take patient to ER. Patients husband did ask we call ER for report and I also gave husband a paper with the vitals and what was given to the patient. This nurse did call ER and spoke to the charge nurse and gave report and let her know patient was on her way.

Medical history

Peripheral neuropathy, Diabetes type 2, antiphospholipid antibody syndrome, Obstructive sleep apnea, lupus, migraines, anemia, anxiety, dyslipidemia, cervical disc herniation, GERD, hypertension, spider varicose veins.

Other medications

Tylenol, aspirin, biotin, vitamin d3, hydroclorothiazide, hydroxychloroquine, ibuprofen, multivitamin, mounjaro, aluminum hydrox-magnesium carb.

Allergies

Sulfa antibiotics, amoxicillin, morphine, hydromorphone, metformin, metoprolol
