Symptom narrative

The vaccine administered had an expiration date of 12/5/25 and was administered on 12/9/25, 4 days post expiration. This was discovered on 12/11/25 when the administering nurse tried to find the vaccine in inventory to complete the patient's chart. The patient was notified and denied any adverse symptoms. The manufacturer was contacted and gave the following response: Per GSK manufacturer in-house testing, the vaccine stability should remain within the potency specifications for 30 days past expiration. However, GSK does not make recommendations regarding re-vaccination or serologic testing following administration of an expired vaccine. CDC recommendations from the ACIP: dose is not considered valid and should be repeated. Immunization registry: As long as the dose was not a live vaccine, it should be repeated as soon as possible. The patient was advised she may return to the clinic to receive a valid dose at no charge to her. She stated that she was not concerned, however if she was able to catch the bus, she would come in for repeat vaccination.
