Symptom narrative

Patient got both covid and flu shot in left arm on 12/11/25. Patient called pharmacy to report arm was very sore and swollen all the way down to his hand. I advised him to go to urgent care. He refused but did allow me to transfer him to a triage nurse at the clinic. I do not know the outcome of his call with the triage nurse. For item 21 I marked none of the above since I do not know the outcome
