Symptom narrative

Patient came to the counter 5 days after receiving vaccine complaining of fatigue and redness of arm where vaccine was given. Patient was asked if there were any other symptoms and was advised to observe if redness increased or decreased as well as the fatigue. Patient came back today and said that he went to the ER/Urgent care because he felt his arm feeling warm still and the facility said that he was experiencing an allergic reaction to the immunization. He was prescribed a 2 day treatment of Prednisone 20 mg by this facility and he picked it up today.
