Symptom narrative

Chills, vomiting, and weakness that was pretty severe in the patient's experience. Patient had trouble getting up form seated position and walking. He fell onto his bed and was unable to get up on his own.

pt had a histoplisty on 11/5/25, had post nasal drip, and a rash
