***patient has not had any adverse effects*** patient came in for their covid booster on 12/8/25. pt was meant to get spikevax 6m-11y but instead was given spikevax 12+. Student gathered vaccine in vaccine room to give vax to pt. when scanning she received an error code stating that the medication is not in the pts wizard. she had asked me to verify what she was doing. As far as I could tell nothing was wrong in that moment. When looking at the vaccine vial there is NO AGE on vaccine for EASY IDENTIFICATION between the two age groups!! this being the reason why this mistake happened.
