Symptom narrative

PCP contacted via facetime by (Reporting/ Referring Provider) on 12/10/2025 to observe adverse reaction. Upon observation. R arm appeared red and swollen. Reporting provider stated, R arm was red, warm to touch, and swollen. Patient complains of pain. VS taken were: BP: 108/55 P:125 02:98%, & T: 103.20. Patient given 2 dual action tablets for fever. After 20 minutes, fever decreased to 102.80. PCP prescribed Keflex 500mg PO every 6 hours for 10 days. Patient seen on 12/09/2025 for wcc and cough. Prescribed Zyrtec 10mg PO once daily. Also patient's sister was seen on 12/09/205, tested positive for Flu A. Possible exposure to Flu A. Mom instructed to make sur patient does not scratch the area, monitor fever every 4-6 hrs for next 3 days, keep adequate hydration status, rest, and give medicine as prescribed. If symptoms do not improve or worsen go to the nearest ER.
