Symptom narrative

Pt came in to receive anthrax. I told him he could receive his TD vaccine. In the process of getting vaccines ready, I grabbed anthrax and flu. Before i gave the vaccines, i repeated what i was giving and administered to the patient. When i looked at the patients record i realized that i had given flu instead of TD. A PSR was put in that same day.
