Symptom narrative

4th dose of hepatitis vaccine given at 9:06. At 9:27 pt stated she felt hot and began taking clothing off, then pt became unresponsive with a pulse. Pt placed in modified trendelenburg with legs elevated above heart and fluids given. Bp at 9:32 100/59 pulse 84. Approx 800 ml fluids given. Pt became responsive by 9:32, but not at baseline. Pt given approx 400 ml fluids due to bp readings of 99/57. Pt c/o of being hot once recovered. Pt recovered on site and left around 1 hour later with bp 116/67 pulse 95 standing bp. Pt stated she feels better. CM and MD notfiied.
