Symptom narrative

Patient reports immediate pain in right shoulder following IM injection. Reports decreased ROM in right shoulder and right arm following injection. Now two weeks later reports she still has sever pain in right deltoid into right trapezius muscle.

anxiety, overweight (BMI 26.54), chronic low back pain, constipation
